Media installed but not mounted?

What can cause this to happen during normal operation? I went to troubleshoot a process that didn't complete and found that no log files had been recorded for 2 weeks. I'm 99% sure that nobody popped out the sd card.I just removed it, and reinstalled and that fixed the fault.
